Mammo-business: un quart de siècle de désinformation (2/4)

Le dépistage du cancer du sein est bâti sur des bases scientifiques douteuses. Il s’est imposé grâce à l’action d’un lobby, qui a usé de moyens non moins douteux pour écarter toute contestation: menaces, ostracisme et dissimulations.

Les programmes suisses de dépistage du cancer du sein informent-ils correctement les femmes âgées entre 50 et 69 ans? Cette population cible est-elle bien instruite des risques qu’elle prend quand on l’invite à effectuer une mammographie tous les deux ans chez un radiologue agréé? Lui expose-t-on honnêtement les bénéfices et les risques de cette démarche? Lui donne-t-on les moyens de prendre une décision libre et éclairée? Le deuxième volet de notre enquête sur le mammo-business montre que c’est loin d’être le cas. Les promoteurs du dépistage dans les cantons helvétiques ont surtout cherché à convaincre les femmes d’adhérer aux programmes «en évitant qu’elles puissent se poser beaucoup de questions», comme le relevait récemment Gianfranco Domenighetti, ancien professeur d’économie et de politique de la santé dans le Bulletin des médecins suisses.

Pour comprendre cette situation, il faut se pencher sur une affirmation martelée depuis des années aux participantes ciblées par les organismes de prévention: le dépistage organisé contribue à abaisser de 25% la mortalité par cancer du sein.

L’histoire de ce «25%» est suédoise. Elle remonte au milieu des années 1980. Le plus peuplé des pays nordiques est en effet le berceau des programmes actuels de dépistage par mammographie: quatre des neuf essais randomisés contrôlés menés sur cette mesure de prévention y ont été réalisés.
